• You're comfortable with your care being managed through a PCP, who determines the need for health care specialists.
• You want the freedom to go out-of-network, and you are willing to pay more of the cost for any care you receive without your PCP’s involvement.
• You don’t want to deal with claim forms—no claim forms are necessary if you use in-network providers.
