Get Divorced or Dissolve a Same-Sex Domestic Partnership/Civil Union?

You have a lot to keep track of during this time of change in your life. Use this checklist to help you keep track of important benefit changes you may want to make. Simply print the list below and check off an item as you accomplish it.

Changes You Should Make Within 31 Days

You can make certain changes online through Your Benefits Resources. Or call 1-800-571-0400 and follow the voice recognition prompts to speak with a Benefits Express representative.

Medical and Dental If you're currently enrolled:
  • You must remove your spouse or same-sex domestic partner/civil union and any step-children as dependent(s) (your dependents may be eligible for COBRA coverage)
If you aren't currently enrolled:
  • Enroll now (for example, if you were enrolled in your spouse's or same-sex domestic partner/civil union's plan)
Group Term Life Insurance Program If you're currently enrolled:
  • Change your designated beneficiary
If you previously waived your coverage:
  • Enroll now
Long Term Disability If you aren't currently enrolled:
  • Enroll now
Reimbursement Account Programs—Health Care and/or Dependent Day Care If you're currently enrolled:
  • Change the amount you contribute
If you aren't currently enrolled:
  • Start contributing

    Note: Costs incurred by a same-sex domestic partner/civil union are not eligible for reimbursement from the Health Care or the Dependent Day Care Reimbursement Account
Group Legal Plan If you're currently enrolled:
  • Change your coverage category to “You Alone” if you have no other dependents you want to cover
Long Term Care If you're currently enrolled:
