Are you ready for NextSteps® Personal Health Improvement Programs?
If you completed the Health Risk Assessment (HRA)* earlier this year, you may have been invited to participate in one of StayWell’s NextSteps® Health Improvement Programs. All of these programs are designed to help participants set and meet goals to improve their health in the following areas:
- Back Care
- Blood Pressure Management
- Cholesterol Management
- Nutrition
- Tobacco Cessation
- Exercise
- Stress Management
- Weight Management
If you qualified for a program you have 3 choices as to how you would like the program administered, by phone, mail or online.
Personalized lifestyle interventions by phone
Participants involved in the phone-based program work one-on-one with a Health Coach over a series of three monthly phone calls at to assess their readiness to change, set goals, discuss barriers and strategies, document progress toward goals and more. This program also includes additional health education resources by mail or e-mail and two follow-up calls to ensure that the participant remains on track with the behavior changes.
Personal-paced lifestyle interventions by mail
The mail-based interventions begin with a personalized welcome and program review that introduces participants to the benefits of committing to behavior change. Participants continue to receive ongoing personalized letters, along with educational brochures and booklets designed to help them set goals, identify barriers and build skills for overcoming them, make progress on new behaviors, strategize self-rewards, and stay motivated over six mailings at one-month intervals with a program evaluation included in the last mailing. The participants can call the StayWell HelpLine at any time to talk to a Health Coach regarding questions on the program or to get additional information.
Personalized lifestyle interventions online
The online programs are six-week, self-paced programs that offer guidance and support in staying healthy or preventing major health problems. Participants receive weekly e-mail “to-do” lists prompting them to go online and work on their health topic area.
